ShareHaven at Evergreen is a Community Based Residential Facility (CBRF) providing 24-hour care for those experiencing symptoms of Alzheimer’s disease and other dementia. Our approach to memory care is built on the foundation of providing personalized care in a safe, comforting and familiar family-like environment. Our highly specialized staff is trained to recognize individual preferences and provide programming to help residents experience pleasant days, while also maintaining their dignity and individuality. We are proud to receive the LeadingAge Wisconsin Dementia Specific designation that recognizes our dedication to a holistic philosophy that embraces and fosters individualized dementia-centered care and support. Memory Care is available to residents of Evergreen on a priority basis, as well as to people living in Oshkosh and surrounding cities.

Evergreen’s Medicare-certified, licensed skilled nursing facility, Creekview Health Center, offers long-term, palliative care and end-of-life services for those needing the highest level of personal care. The Creekview Health Center features an innovative, nationally recognized household design with living spaces much like residents’ previous homes. Long-term care services are available to residents of Evergreen on a priority basis, as well as to people living in Oshkosh and surrounding cities. Creekview Health Center is made up of 6 households, that includes 34 private and 12 semi-private rooms with private half-baths, with 9-11 residents per household.
