As an athlete, keeping your edge is important to you. As your local health care provider, your health is important to us. Under the Health Care Reform Act, you are entitled to receive your yearly physical free of charge with your primary care provider. However, if you wish to schedule a sports physical, you may do so at any time for a $25 fee payable at time of service. This sports physical meets school and WIAA requirements. Students should dress in shorts and gym shoes and are required to bring their completed WIAA sports card. A parent or guardian must accompany the athlete, or print and sign this consent form.
For those diagnosed with Early Memory Loss and for care partners of people with memory loss in any stage. These are support-focused groups for those diagnosed with Alzheimer’s or related dementia in the early or mild stage, and for care partners to those with dementia in any stage. Groups meet at the same time and in the same location, but separately from each other. Each meeting will start with a topic for discussion led by Dementia Specialist/Certified Care Manager. Time will also allow for opportunity to share your current experiences, concerns, or interests.

SPARC is an on-the-job training program for future CNAs, CMAs, LPNs and RNs exclusively at DCMC.
The Certified Nursing Assistant (CNA) class offered by Door County Medical Center (DCMC) in Wisconsin provides students with the knowledge and skills necessary to become a certified nursing assistant. The class is taught by experienced medical professionals and includes both classroom instruction and hands-on training. Upon successful completion of the class, students are eligible to take the Wisconsin CNA certification exam.
